OneTrue are recruiting for a committed Social Worker to work full time within the Adults Integrated Team based in London Borough of Brent. The ideal applicant will have previous experience working in the same or a similar service, be very dedicated, personable and willing to contribute positively to the team and service to ensure the best and most appropriate outcomes are delivered. Applicants must have previous experience working with UK statutory services and hold valid HCPC registration.

 

The pay rate for this Social Worker role is £28.00 + £0.72 AWR (Umbrella Company) per hour and is an ongoing contract.

Requirements of the Social Worker

* Degree or equivalent in social work

* Current HCPC registration

* Experienced Social Worker in respect of working with Physical Disabilities and Older People in Rehab and Reablement setting. Able to set goals and monitor performance

* Experience of undertaking mental capacity assessments and working collaboratively across a wide range of services and with NHS partners

* Experience of undertaking complex assessments and drawing up complex care and support plans including carers assessments

* Experience of working with home first/discharge to assess model of practice.

 

Specific Role Information

This post is for an experienced Social Worker to be working in an integrated Team alongside NHS Rehab OTs and Physios. It is for 3 months to cover a short term absence.

 

Undertaking Reablement Goal Setting Assessments and Reablement reviews with clients, their family/ carers and external Reablement care providers.

Completing Social care Assessments and Support plans to present for authorisation for funding

Working with colleagues across health, social care and housing to ensure a collaborative and person focussed service

Maintaining accurate case and data records.
