The ideal applicant will have previous experience working in the same or a similar service, be very dedicated, personable and willing to contribute positively to the team and service to ensure the best and most appropriate outcomes are delivered. Applicants must have previous experience working with UK statutory services and hold valid HCPC registration.
The pay rate for this Social Worker role is £19.30 + £0.42 AWR (Umbrella Company) per hour and is an ongoing contract.

Requirements of the Social Worker
* Degree or equivalent in social work
* Current HCPC registration
* Ability to manage risk to individuals, families, carers, groups, communities, self and colleagues
* Able to support individuals to represent their needs, views and circumstances
* Demonstrate professional competence in social work practice
* Manage and be accountable, with supervision and support, for your own social work practice within your organisation.
Specific Role Information
An opportunity has arisen for a social worker to join Caerphilly Children's Services 16 Plus Team.
The 16 Plus Team are looking to achieve the best long-term outcomes for the young people they work with, The Team strives to support young people making their transition into adulthood an easier and more manageable experience.
As a Social worker within the 16 Plus Team you will be responsible for working in partnership with young people, their carers and other professionals to complete high quality assessments in order to develop pathway plans. The Team also has responsibility to assess the needs of young people age 16/17 who present as Homeless to the department.
You will also be required to contribute to service developments and occasionally take the lead on arranging meaningful activities that will benefit young people. You will assist other staff in developing and facilitating group work sessions when particular issues arise.
You will need to have a genuine interest in working with teenagers and be able to communicate with them regardless of their situations.
In turn you will become part of an established team. You will receive regular supervision support and guidance and become part of a motivated and committed Team.
To undertake high quality assessments with eligible/relevant and homeless young people and to develop knowledge and skills to provide a quality service to young people transitioning to the 16 Plus Team.
To develop, maintain and review Pathway Plans for young people and ensuring plans for these young people are achieving the identified outcomes.
To develop and maintain effective Pathway Plans for Looked After young people, in partnership with themselves, their carers and family. In order to Safeguard their welfare and ensure that these plans are fully implemented and regularly reviewed.
To attend LAC reviews and other relevant planning meetings for young people.
To maintain accurate case records in accordance with the Directorate policy, keeping such records up-to-date, and ensuring that information about work on the Service Plan is accessible and understandable to others.
To develop and participate in groups for young care leavers.
To work in partnership with other agencies to ensure best outcomes for young people.
To prepare reports and information as and when required.
