OneTrue are recruiting for a committed Social Worker to work full time within the Countywide and Out of County Adult Social Care Review Team based in Oxfordshire. The ideal applicant will have previous experience working in the same or a similar service, be very dedicated, personable and willing to contribute positively to the team and service to ensure the best and most appropriate outcomes are delivered. Applicants must have previous experience working with UK statutory services and hold valid HCPC registration.

 

The pay rate for this Social Worker role is £27.50 - £36.00 (depending on experience) (Umbrella Company) per hour and is an ongoing contract.

Requirements of the Social Worker

* Degree or equivalent in social work

* Current HCPC registration

 

Specific Role Information

Locum social worker to be part of a Countywide and Out of County Adult Social Care Review Team. We work with a diverse service user group who are in receipt different service provisions (ie. DP, Supported Living, Care Home, PoC).

Targeted approach to engage in completing current assessments of need against Care Act 2014 criteria, looking at how support is designed; taking a strengths based approach to practice utilising alternative resources and equipment.

SMART support planning required.

Scrutiny and checking of financial arrangements and contributions being made.

Expectation of a caseload of 20 at any given time with throughput of 2-3 re-assessments weekly.

Assess and review packages of care

Rightsize service provision and make savings where overprescribed or improved level of ability.

Case recording, assessment writing, support planning, informed decision making, completion of relevant checklists and documentation.

Role of ensure advocates are used as appropriate.
