The ideal applicant will have previous experience working in the same or a similar service, be very dedicated, personable and willing to contribute positively to the team and service to ensure the best and most appropriate outcomes are delivered. Applicants must have previous experience working with UK statutory services and hold valid HCPC registration.


The pay rate for this Social Worker role is £27.39 (Umbrella Company) per hour and is an ongoing contract.

Specific Role Information

An experienced and dynamic social worker is required to develop the discharge planning process for patients leaving Alderney Hospital in Poole.

The social worker must have experience of discharge planning from hospital (in particular older person's mental health). They must also have good understanding and experience of applying the Mental Capacity Act, DoLs, CHC processes, and Mental Health Act in relation to discharge planning.

Experience of multi-disciplinary working within a hospital setting and the ability to maintain a person centred, strengths based approach is essential.
